Medicare and Medicaid certify 14,686 US nursing homes with 1,567,009 beds. In the three years to October 1, 2026, they were fined $445,738,479 across 12,638 fines and had 2,369 payment denials. 2,753 homes hold one star, and CMS lists 332,420 health citations from inspections in that time.
